Three Wishes is a short, whimsical visual novel in which you guide three idol little-sisters through a string of increasingly absurd wishes. On their way to a sleepover, Addie, Mint, and Emi stumble across a mysterious girl named Saty, who claims she can grant only three wishes. Your choices determine what each girl asks for, and every wish stacks onto the last, sending the story into ever-more unexpected and comedic directions. A full playthrough takes roughly 3-4 hours and there are over a dozen endings to discover.

The game leans into playful, experimental mechanics:
- Headpats Engine: Characters on screen are interactive-tap to give headpats at nearly any moment, and watch their reactions play into the scene.
- Props & Effects: Routes are packed with visual gags, props, and scene-specific effects that keep every route lively and surprising.
- Quips System: Side reactions and little bits of dialogue appear alongside the main lines, letting characters react in real time and adding extra personality to conversations.

There is no sexual content, though some scenes include cartoon blood, depictions of death, and moments some players may find disturbing.
