UNDERWARD is a tense horror-exploration game for one to four players. You and your teammates descend into a long-abandoned underground hospital to recover mysterious "TEST-SUBJECT" capsules and carry them back to the elevator. Twisted corridors, hidden traps and lethal nurse-like monsters lurk around every corner, demanding stealth, teamwork and quick thinking.

In multiplayer sessions, communication and cooperation are essential. Use proximity-based voice chat to warn each other of danger, split up to search lockers and shelves for useful gear, and work together to perform a "Connected Carry" that lets you haul heavy capsules faster than the pursuing creatures. If someone is caught, their location appears on your HUD-rescue them before time runs out, or your entire team faces a wipeout.

A single-player mode offers an ultra-high-difficulty stealth challenge: you’ll have extra lives and balance tweaks, but getting spotted means instant game over. Without the ability to chain carries, each step feels treacherous as you haul your cargo alone. You’ll learn through trial and error, avoiding detection, disarming traps and adapting to a truly unforgiving environment.
