"3rd Eye" is a 2D exploration and horror game featuring Koishi Komeiji from the Touhou Project. In this game, players use Koishi's Third Eye to navigate a mental world that defies reality. The Third Eye allows players to discover hidden elements within this world, but overuse causes it to become bloodshot, requiring eyedrops to prevent it from turning completely red.

Koishi Komeiji, the protagonist, has the ability to read others' thoughts. Due to fear of her Third Eye, she has lived a secluded life, isolated in her room. Her family is absent, with her sister having left her alone. One night, Koishi finds herself in an unfamiliar room, marking the beginning of a transformative journey.

The game features several characters, including Koishi's older sister Satori Komeiji, who prefers the company of animals over people. Other characters include the Rabbit Girl, who is stuck in a job she dislikes, and the Girl in Red and the Girl in Blue, both of whom believe they can achieve anything. Reisen Udongein Inaba, a mature woman who dispenses medicine, also plays a role in the story.

Players explore various locations such as The Park, where they can interact with shopkeepers and working children, and The Snowfield, a vast snowy landscape where treasures might be hidden. The Bamboo Thicket offers a sense of emptiness, while The Mansion serves as a home where players can engage with family members, provided they adhere to the rules.
