Saiko No Sutoka No Shiki is a spin-off of the original Saiko No Sutoka game, focusing on survival horror elements where you must escape from AI characters that hunt you in unique ways. The game introduces three new AI with distinct behaviors, each using the forest environment to their advantage.

The first antagonist is Yui, a new character who is hostile towards the player. She attacks with scissors and uses trees to ambush you. Yui also taunts you to lower your morale and ultimately aims to kill Saiko. To escape, you must defeat Yui, as she holds the key to the exit gate.

Saiko, from the original game, returns as a zombie. You need her help to defeat Yui, but first, you must ensure Saiko becomes stronger by completing her parts. Be cautious, as Saiko can get hungry and might turn on you if not fed.

Elissu, previously an easter egg, now has her own mode with a revamped AI suited for the forest map. Her AI operates in three phases: Stalking Mode, where she silently follows you; Playful Mode, where she interacts without lethal intent; and Kill Mode, where she becomes deadly and uses various methods to eliminate you.

The game features a combat system allowing players to use melee weapons and block enemy attacks. The forest map is randomly generated with three variations, making navigation challenging. The AI uses trees and bushes to hide and stalk the player.

Saiko No Sutoka No Shiki offers multiple endings, with four in total. These endings are interconnected, providing a comprehensive conclusion to the story. To fully understand the spin-off, multiple playthroughs are necessary.
