The story follows Riley, who is dealing with more than just a world in chaos. Three months ago, Riley's sister took a job with the Osiris Initiative, a leader in medical technology. Initially, they had a way to communicate, but contact has since ceased, raising suspicions. After being fired for trying to use her job connections to find her sister, Riley decides to become a test subject for the Osiris Initiative, hoping to uncover information about the elusive organization and find her sister. The safety of the experiment, however, is uncertain.

Key features of the game include exploring a vast complex, uncovering the secrets of the experiment, and attempting to escape alive. Players will solve puzzles using items found during their isolation and encounter a long-lost piece of civilization that may not be as civil as it seems. The game allows exploration in first person using the camera and puzzle-solving in third person. Additionally, multiple endings are available based on player choices.
